INDIVIDUALS
All students participating in the club sport program at La Salle University must complete the Medical Clearance, Hazing Contract and Facilties Waiver. The Hazing Contract and Facilities Waiver can be complete online, but the Medical Clearance must be stamped by a physician and turned in to the Recreation office. Medical Clearances can be faxed to 215.951.5127.
