PHYSICALS
All members of the cheer and dance teams will be required to submit a physical prior to participation, including tryouts. A health history form will need to be filled out prior to reporting to campus for your physical, and all athletes are required to sign up for or log on to their SportsWare Online account. Instructions for this process are below:

NOTE: Per NCAA guidelines, physicals must be completed within 6 months of date of first team practice/tryout. Physical forms must be given to the Athletic Training Office in person during normal business hours or mailed to the address below.
